First City has been awarded the Lead Provider contract for North and East Isle of Wight by the Isle of Wight Council, providing, or brokering, domiciliary care for the residents within the locality. These clients include those identified to be medically fit to be discharged from the local acute hospital.
To facilitate the transfer of care from the community andthe acute setting First City are establishing a brokerage function on the Island. The postholder will work closely with our established commissioning and contract team based at head office.
The postholder will also broker packages of support in Swindonwhere we also deliver a lead provider contract. The post holder will also be responsible for managing invoice queries relatingto brokerage across IOW and Swindon working closely with the finance department.
The brokerage team will source packages of care from the partner organisations First City will subcontract with. In addition to this the brokerage officer will be responsible for working across the system to ensure there is sufficient capacity to meet the needs of the locality.
SPECIFIC RESPONSIBILITIES
Professional
* Broker packages of care to partners
* Maintain supply across the marketplace, coordinatingchanges to packages and identifying emerging capacity
* To represent First City within the acute setting
* Ensure service users needs can be met by proposed package of care
* Work within policies of First City
* Demonstrate decision making based onevidence-based practice
* Act as patient advocate to ensure safe, timely, transfer of care within the community
* Support with assessments and support planning activity
* Support with contract management and quality assurance
Management
* Provide support to wider members of brokerage team as required
* Identify adverse incidents and work with appropriate agency to report and identify learning opportunities
* Contribute to risk assessments for service users to ensure their, and staff, safety
* Ensure safeguarding processes and procedures are followed at all times, support with the ongoing development of a collaborative approach to safeguarding practice.
Communication
* Participate in regular supervision as dictated by line manager* Contribute effectively to team working in a professional and advisory capacity
* Ensure professional, concise, thorough entries in patient / service user records
* To effectively communicate sensitive, complex information to the receiving team in accordance with all relevant data protection legislation
Service development
* Work with wider brokerage team to ensure compliance with KPIs ( Key Performance Indicators )
* Contribute to regular contract review meetings to further develop role* Promote effective relationships between First City / community health/ primary health care and Isle of Wight Council plus subcontracted providers
General
* This job description is not exhaustive and can be altered in consultation with the post holder.
* Any additional duties which may be requested by the line manager in order to facilitate the smooth running of the service
* Position is Swindon and remote working

Working hours are Monday - Friday 08.30am -17.00pm. However, some flexibility in hours may be required tosuit the needs of the business.
