Description:
We are currently seeking a Care Coordinator to join our expanding workforce in Swindon. This is a permanent opportunity, working within a friendly, supportive team. The ideal role for someone looking to take the next step in the health and social care career.
Location: Swindon
Salary: £29,270 OTE
Hours: 40hrs per week + out of hours shared on call service (Office hours Monday-Friday 08:30am -17:00pm. Flexibility in hours maybe required to meet the need of the business)
Duties can include:
* Allocation of care workers to customer visits usingeffective rostering, ensuring continuity of care and considering locations, skills required.
* Communicating regularly with care workers, providing guidance and resolving queries in a timely manner
* Liaison with other healthcare professionals
* Ensure all paperwork is completed and entered onto relevant systems
* Maintain integrity when dealing with confidential matters* Approach clients with understanding and empathy, remembering that each client is to be treated as an individual
Essential for role:
* Experience of working in health and social care (within a community-based setting would be preferable)
* Be computer literate and have good office skills
* Be Honest, Caring, Non-judgemental and to act with Kindness and Respect
* Be organised and work calmly under high pressure
Desirable:
* Health and social care qualifications (NVQ 2/3 or above)
* Valid driving licence with access to a car
