First City nursing services have a vacancy within the SHAPE (Strength-based, holistic, approach/aspirations/assessment, promoting, enablement) teamfor someone who is truly passionate about providing high standards of outcome focused, person centred, strength based care in the community and at home. As a SHAPE facilitator you will be responsible for the assessment and writing of strength based care plans, risk assessing, MAR detailing, Auditing and compliance.

In this role you will be working closely with the client and their care team to create and tailor their own package of care. You will be required to think outside the box about alternatives toformal care services and understand how to utilise local services and facilities to deliver a truly person centred service thatis compliant with standards set by CQC.

As a SHAPE facilitator you will be required to attend assessments and rebuilds withthe client and their families and liaise with a variety of professionals across the health sector. First city works closely witha vast array of clients and services so adaptability is a must.

For this role you must have a background of at least 12 months in care delivery and hold a level 2 qualification, equivalent or above in health and social care. Key Skills you should possess:

* Good English and computer skills are important
* Excellent communication skills
* A professional manner supportinga 'can do' approach to work.
* Applicants must be flexible and reliable with the ability to work alone and as part of a team.* Be able to problem solve and manage work loads in a high pressure environment.
* Driving license and use of personal vehicle for work purposes is essential for this role
