Childrens Continuing Care Case Manager. NHS Trust
£190 per day

Job Summary
The post holder will ensure that health care needs of children eligible for Continuing Care are identified and met in accordance with the National Framework. They will work as part of a multidisciplinary team with external agencies to ensure children's health care needs are supported appropriately. The case manager will carry out assessments of need, health care reviews and attend statutory meetings for children with life limiting conditions and complex health needs, supporting families with care packages, ensuring they are supported appropriately to continue to care for their child in their family home.And ensure that they always adhere to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures and protocols ensuring quality standards and performance as directed by the Children's Clinical Lead and Head of Continuing Healthcare Clinical Services.
Experience
Essential:
* Experience caring for children with complex needs.
* Good knowledge of NHS and Social Services guidance and Continuing Cares national framework.
* Evidence of mentorship.
Desirable:
* A background that demonstrates a clinical experience in a children's healthcare setting including inter-agency and multi-agency.
* Able to demonstrate strong knowledge base around safeguarding children and consent.* Clinical knowledge base in order to ensure scrutiny of compliance with statutory guidance and best practice.
* Clinical knowledge base in order to ensure comprehensive holistic assessment for Continuing Care.
* Experience of managing risks and reporting.
* Experience of managing the performance of a team without supervision.
Qualifications/Skills
Essential:
*Active registration as a registered Children's Nurse.
* Degree in a health-related subject or equivalent experience that demonstrates critical and analytical thinking.
* Holds a UK driving licence and has access to a vehicle insured and taxed.
* Knowledge of adult and child safeguarding process and responsibilities.
* Aware of Caldicott principle and information governance.* Understanding of Confidentiality and Data Protection Act.
* Understanding of the public sector.
* Awareness of equalityand valuing diversity principles.
Desirable:
* Master's in healthcare related area or equivalent level of knowledge andexperience in relevant subject area.
* Other relevant specialist post registration qualification, experience
* Expert knowledge of Continuing Care pathway including screening, assessment and decision-making processes, review and current appeals.
