Description:
An exciting position has arisen for Advanced Nurse/Clinical Practitioner with a keen interest in urgent care development to join a well established Group within Leeds, West Yorkshire.
As an experienced ANP/ANP, you will utilise your expert clinical knowledge to manage patients presenting with acute problems in addition to those with long term chronic conditions.
Working across Urgent Care Services in Leeds, the ANP/ACP will manage acute presentations to the Walk-In Centre and at Leeds General Infirmary and St James primary care services, as part ofa team of autonomous Practitioners; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Emergency Departments in Leeds (streaming).
To be considered, the following are ESSENTIAL:* First Level registered nurse (NMC) ECP, or ACP
* MSc in advanced Clinical Practice or Relevant ANP courses at level 6 or7, or equivalent in Advanced Physical Assessment skills, Minor Illness, disease management, etc.
* A Non-medical prescribingqualification
* Evidence of continued professional development.
* Teaching qualification i.e., 998 or equivalent (Desirable)
* Satisfactory DBS clearance
* Experience of managing acute undifferentiated presentations
* Ability to triage (desirable)
* Good understanding of autonomous practice within a minor illness/urgent care centre and the roles that deliver this
* Good interpersonal and communication skills.
* Experience in using I.T. based patient documentation systems.
* Knowledge of IT systems including System One, Adastra or EMIS (desirable)
* Ability to manage and prioritise workload.
* Proven ability towork as part of a team.
* In-depth Knowledge of the CQC Key lines of Enquiry
* Current knowledge of government targets andinitiatives and N.I.C.E. guidelines and their impact on clinical practice.
* Understanding of clinical audit.
* Actively supports Clinical Governance
* Acts as a mentor for other clinical staff as required
