Job Summary
The post holder will be part of a service which aims to increase Mental Health capacity andexpertise in Primary Care. As a Primary Care Mental Health Specialist, you will be expected to develop, plan, coordinate, deliver and evaluate the service provided in primary care setting within the East Kent area. The post holder will need to be flexibleand responsive according to the needs of the service as well as work as part of a wider PCMHS team across Kent. The post holderwill work as an autonomous practitioner, responsible for the provision of assessments, interventions, education, and discharge planning of patients referred from within the East Kent area. They will have responsibility for managing a caseload of clients with a variety of Mental Health conditions and will be expected to collaborate with other health care professionals including GP's, Practice Nurses, Secondary Care practitioners and third-party organizations. If applicable, you will be responsible for the administration of depots within clinical settings and supervising junior staff such as the HCA who will be carrying out physical health checks with patients.
Responsibilities Key dimensions of the role include:
Support practice staff in the management of Mental Health conditions in Primary Care.
Support the effective 'navigation' of patients through the various mental health systems and services.
Ensure that local voluntary sector, health, and social care provisionare used effectively.
Provide assessment, follow up, therapeutic treatment groups and/or depot clinics for patientsseen within primary care.
Support recovery by pro-actively enabling medically stable individuals to move from secondary to primary care.
Facilitate treatment groups in keeping with NICE guidelines and specific conditions in PrimaryCare.
Support Primary Care initiatives which improve the physical health outcomes of those with a long-term Mental Health condition and the mental health outcomes of those with a long-term physical health condition.
To work as Primary Care Mental Health Specialist in a variety of community settings where there is a requirement to make decisions spontaneously, within local policies and guidelines.
