Description:
Key Responsibilities
* Conduct comprehensive assessments of patients to determine their occupational needs and goals related to their medical conditions.
* Develop and implement individualised treatment plans in collaboration with patients and multidisciplinary teams.
* Deliver therapeutic interventions to improve patients' functional abilities and promote safe discharge planning.
* Educate patients and their families about strategies to manage daily activities and support rehabilitation efforts.
* Collaborate closely with nursing staff, physiotherapists, and other healthcare professionals to ensure a coordinated approach to patientcare.
* Monitor patient progress and adjust treatment plans as necessary based on clinical outcomes.
* Maintain accurate andup-to-date patient records in compliance with departmental policies and relevant regulations.
* Participate in team meetingsand contribute to quality improvement initiatives within the service.
* Provide guidance and support to junior staff and therapy students as required.
Qualifications and Experience
* Degree in Occupational Therapy and current registration with HCPC.
* Experience working in an acute hospital setting, particularly within a medicines division or related area.
* Strong clinical reasoning and assessment skills, with the ability to work effectively in a fast-paced environment.
* Excellent communication, teamwork, and interpersonal skills.
* Knowledge of discharge planning processes and rehabilitation principles.
