MyClient are an NHS Trust based in Lincoln and are seeking a Children's Continuing Care Case Manager on an initial 3 month contract basis with (30 hours a week) paying £230 per day.
The role will be based at Cross O'Cliff, Bracebridge Heath, Lincoln, LN42HN - with a mix of face to face and teams assessments across Lincolnshire.
Job Summary
An exciting opportunity has arisen for a motivated compassionate children's nurse to join Children's Continuing Care, driven to transform current practice and make a positive change to children and their families. Children's Continuing Care are looking for a children's nurse experienced in caring for children with complex health needs or learning disabilities.
The post holder will work as member of the NHS Lincolnshire Integrated Care Board Children's Continuing Care Team, working to improve the outcomes for health care in Lincolnshire.The Childrens Continuing Care Case Manager will have excellent clinical knowledge as well as communication and assessment skills; be resilient and able to work autonomously but also work as part of a team. They will ensure that health care needs of children eligible for Continuing Care are identified and met in accordance with the National Framework. They will work as part of amultidisciplinary team with external agencies to ensure children's health care needs are supported appropriately. The case manager will carry out assessments of need, health care reviews and attend statutory meetings for children with life limiting conditions and complex health needs, supporting families with care packages, ensuring they are supported appropriately to continue to care for their child in their family home.
The Children's Continuing Care Case Manager will ensure that they adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures and protocols ensuring quality standards and performance as directed by the Children's Clinical Lead and Head of Continuing Healthcare Clinical Services.
Experience
* Experience caring for children with complex needs.
* A background that demonstrates a clinical experience in a children's healthcare setting including inter-agency and multi-agency.
* Good knowledge of NHS and Social Services guidance and Continuing Cares national framework.
* Able to demonstrate strong knowledge base aroundsafeguarding children and consent.
* Evidence of mentorship.
* Clinical knowledge base in order to ensure scrutiny of compliance with statutory guidance and best practice.
* Clinical knowledge base in order to ensure comprehensive holistic assessment for Continuing Care.
* Experience of managing risks and reporting.
* Experience of managing the performance of a team without supervision.
Knowledge
* Evidence of continued professional development
* Knowledge and understanding of the national policies and guidance relevant to the National Framework for Continuing Care.
* Expert knowledge of Continuing Care pathway including screening, assessment and decision-making processes, review and current appeals.
* Knowledge of adult and child safeguarding process and responsibilities.
* Aware of Caldicott principle and information governance.
* Understanding of Confidentiality and Data Protection Act.
* Understanding of the public sector.
* Awareness of equality and valuing diversity principles.
Qualifications
* Active registration as a registered Children's Nurse.
* Degree in a health related subject orequivalent experience that demonstrates critical and analytical thinking.
* Holds a UK driving licence and has access to a vehicle insured and taxed.
